# Testosterone Undecanoate (Oral)

*Also known as: Jatenzo, Andriol, Oral TU*

Oral testosterone (Jatenzo/Andriol) with low lymphatic bioavailability; dosed twice daily with food.

## Overview

Oral testosterone undecanoate (Jatenzo, Andriol) is the approved oral testosterone preparation, used for testosterone replacement therapy in men who cannot or will not use injectable or transdermal forms. The undecanoate ester is absorbed intact through the intestinal lymphatics alongside dietary fat, which bypasses much of the first-pass liver metabolism that destroys other oral testosterone formulations. The compound has a roughly 18-hour half-life, peaks within hours, and shows very low bioavailability near 7%, so it must be dosed twice daily with a fat-containing meal to maintain serum levels. Effects include restored testosterone without injection, while the formulation carries a boxed warning for blood pressure increases and cardiovascular risk, and dosing is sensitive to meal fat content and dosing interval. Harm reduction centers on taking it with food, monitoring blood pressure closely at baseline and during titration, and scheduling doses evenly to avoid the rapid troughs the short half-life produces.
