## Overview

Sublingual testosterone is an ester-free testosterone base preparation held under the tongue, delivering testosterone directly into systemic circulation through the oral mucosa. Bypassing much of the intestinal first-pass metabolism that destroys swallowed testosterone, the sublingual route produces an extremely rapid serum spike followed by a fast fall, mimicking the pharmacokinetics of a short pulse rather than sustained replacement. The compound has a very short roughly 36-minute terminal half-life, peaks within roughly 14 minutes, and shows about 30% bioavailability, so dosing two to three times per day is required to approximate physiological testosterone levels. Effects include rapid onset suited to pre-dosing for specific timing or as an adjunct to longer-acting forms, while the very short half-life produces sharp swings in serum testosterone, estradiol, and mood across the day. Harm reduction centers on the understanding that multiple daily doses are needed to avoid large troughs and on monitoring serum testosterone at expected peak and trough given the wide daily variation.
