## Overview

Tesamorelin (Egrifta) is an N-terminally stabilized synthetic analog of growth hormone-releasing hormone (GHRH) that is FDA-approved to reduce excess visceral abdominal fat in HIV-associated lipodystrophy. It works by stimulating endogenous growth hormone release in a pulsatile pattern, which in turn shifts body composition away from visceral fat accumulation. Unlike most compounds on this list, it carries a real approval and a solid clinical evidence base, with Phase 3 trials demonstrating meaningful visceral fat reduction in the target population. Its half-life is short, roughly 30 minutes, with around 50% subcutaneous bioavailability, which drives daily subcutaneous dosing.
