## Overview

Mazdutide is an investigational once-weekly peptide that acts as a dual agonist of the GLP-1 and glucagon receptors, in development for obesity, type 2 diabetes, and non-alcoholic steatohepatitis. By co-activating GLP-1 receptors, it drives insulin release, slows gastric emptying, and reduces appetite, while the glucagon receptor activity adds direct hepatic fat oxidation and thermogenic effects that single GLP-1 agents lack. The compound has a very long roughly 434-hour half-life, peaks within roughly 3 days of subcutaneous injection, and shows about 80% bioavailability, which is what allows once-weekly dosing and produces steady exposure over several months. Phase II data show substantial weight loss exceeding many current GLP-1 comparators, while the main adverse effects are gastrointestinal, including nausea, vomiting, diarrhea, and reduced appetite, consistent with the incretin and glucagon activity. As an investigational agent with no approved use, harm reduction centers on the understanding that long-term safety data are still maturing and on gradual titration to manage gastrointestinal tolerability.
