## Overview

Estradiol valerate (Progynon) is an injectable estradiol ester used in feminizing hormone therapy and menopausal hormone replacement. The valerate ester is cleaved by tissue esterases to release free 17-beta-estradiol from the depot, with a release rate that sits among the shorter of the injectable estradiol esters and supports dosing every 5 to 7 days. The compound has a roughly 84-hour half-life, complete apparent bioavailability by injection, and accumulates modestly over the first weeks of use. Effects include sustained parenteral estradiol exposure without the first-pass metabolic burden of oral dosing, while the main practical issues are the relatively sharp peak compared to longer esters and the risk of supraphysiological levels if dosing is pushed. Harm reduction centers on titrating to estradiol bloodwork drawn at trough, on spacing injections to avoid cumulative peaks, and on monitoring estradiol alongside liver function and clotting risk factors during therapy.
