## Overview

Cabergoline is a long-acting ergot-derived dopamine D2 receptor agonist used to treat hyperprolactinemia and prolactin-secreting adenomas, and used off-label to manage prolactin elevation sometimes associated with 19-nor compounds. It directly stimulates pituitary D2 receptors, inhibiting prolactin secretion, and its ergot-derived structure gives it high potency and a prolonged duration of action. The compound has a roughly 86-hour half-life, supporting twice-weekly oral dosing, with peak plasma within a few hours and roughly 45% bioavailability, and prolactin typically normalizes within days of starting. The main documented risks with long-term or higher cumulative dosing are cardiac valve fibrosis and impulse control disorders, so conservative dosing and periodic prolactin bloodwork are central. Harm reduction centers on confirming elevated prolactin before treating rather than dosing presumptively, on using the lowest effective dose, and on monitoring for impulse or mood changes and stopping if they appear.
