## Overview

Aromasin (exemestane) is a steroidal, irreversible aromatase inhibitor developed for estrogen-receptor-positive breast cancer and used off-label to manage estradiol during testosterone therapy or anabolic cycles. Unlike reversible inhibitors, it is a mechanism-based, or suicidal, inactivator that permanently binds and destroys the aromatase enzyme, so recovery of estradiol requires new enzyme synthesis over several days. The compound has a roughly 23-hour half-life but very low oral bioavailability near 5%, and steady estradiol suppression builds over repeated dosing. Because the enzyme is permanently inactivated, the estradiol fall can overshoot quickly, and the dominant risk is over-suppression, with joint pain, libido loss, lipid deterioration, and mood disturbance when estradiol runs too low. Harm reduction centers on conservative, symptom-driven dosing guided by estradiol bloodwork rather than fixed scheduling, avoiding overlap with other aromatase inhibitors, and retesting promptly if joint or libido symptoms appear.
